Audience: LEVRG independent contractor sales representatives submitting forms in the Documents hub.
Consent to electronic signatures and records
By checking the electronic signature consent box when you submit an internal contractor form in the LEVRG application, you agree that:

Intent to sign electronically. Typing your full legal name in the signature field is your electronic signature and has the same legal effect as a handwritten signature for that submission, to the extent permitted by applicable law (including the U.S. Electronic Signatures in Global and National Commerce Act and state Uniform Electronic Transactions Act implementations).

Consent to conduct business electronically. You consent to receive, review, and sign the form and related records electronically through the LEVRG platform rather than on paper.

Hardware and software. You confirm you can access this disclosure, the form you are submitting, and a copy of your submission confirmation with standard web browser software and a device you control.

Withdrawal of consent. You may request a paper copy or withdraw consent to electronic transactions for future submissions by contacting support@levrg.ai. Withdrawal does not affect the validity of submissions you already completed electronically.

Relationship to other agreements. This disclosure applies to in-app contractor form submissions. It does not replace your separate contractor agreement, commission plan, or LEVRG Terms of Service and Privacy Policy, which govern your overall relationship with LEVRG.
